Heartwood Care Group have been established for more than a decade, supporting children and young people in the community, delivering an individual and flexible service for those with emotional and/or behavioural difficulties who are emotionally vulnerable and sometimes challenging. We support them to achieve their goals and reach their full potential as they move-on from children's services to adult life. We have gained a great deal of experience over the years in caring for children and young people from the time they come into our residential children's homes from the age of 11 and we are passionate about continuing to support the children and young people through the transition phase in our 16+ semi-independent living services and Outreach services for young people aged 18+. Through our dedicated staff teams we help them to develop the confidence and independent living skills they'll need as they move on to adulthood. For those young people over the age of 16 who require more specialist support, we offer bespoke, tailored solutions, led by our own behaviour specialist.
