Founded in 1979, the Foundation for Senior Care's mission is to provide programs and resources enabling seniors to enhance their well-being and give them a more meaningful life. We are located in Fallbrook, a quiet, rural community in Northern San Diego County (approximately 55 miles from the city of San Diego) surrounded by avocado groves, citrus groves and horse farms. Our service area extends throughout the rural communities of Fallbrook, continuing northward toward the Temecula region and southerly toward Escondido, and Oceanside. We provide four interrelated programs that assist seniors and the disabled stay active and continue to live independently. The Care Van program was established in response to the lack of accessible and affordable transportation. Annually, our Care Vans provide over 8,000 rides for seniors and disabled adults to healthcare appointments, grocery stores, pharmacy, and other local businesses. Our Care Advocacy program ensures clients have access to local resources and national entitlements to manage their healthcare, maximize their independence and enjoy an enriched quality of life. We seek to reestablish healthy family connections, educate loved about the severity and prognosis of situations related to aging, and seek practical solutions. Last year our Care Advocates provided over 3,500 points of contact, including home safety visits and medical care coordination. Senior Care Computer Learning Center, staffed entirely by computer savvy senior volunteers, offers computing classes, Fix-it Fridays, and workshops to seniors so that they can get and stay connected to family and the world. Lastly, the Fallbrook Adult Day Care Center, known as ‘The Club', which offers a secure social, recreational day program designed for seniors and disabled adults that need additional help. Besides providing stimulation and socialization to our Club clients, this day program provides caregivers with a wonderful respite opportunity giving them some downtime to rejuvenate.
