Home and Community Based Services (HCBS) Sunny Days staff will come to you or a loved one in the comfort of your own home to provide the assistance necessary to maintain your lifestyle. Whether it is helping with meals, shopping, or providing a companion when a caregiver is away, Sunny Days provides trained care and supervision in the comfort of your own home. Aging In-Home Services include:• Personal Care• Chore/Housekeeping• Companionship• Respite• Specialized Care Day Services/ Community Participation (CPS)Our day program facility provides tailored programming for individuals with physical and/or intellectual disabilities, as well as aging adults who need companionship and support during the day. Participants have the opportunity to go on outings into the community to build personal connections with the community surrounding us. Day/CPS Services include:• Companion• Respite• Community-Participation SupportsResidential ServicesEach individual has their own bedroom and share common areas with their housemates. Staff on sight 24/7/365 to provide assistance with daily living needs, medication reminders or administration, personal hygiene tasks, personal goals, transporting to doctors, work, shopping, and desired community events.
