Founded in 1993 as the Adult Day Services Network of Alameda County (ADSNAC), DayBreak is a leading non-profit network of 13 adult care centers located across Alameda County. The member centers provide specialized health and wellness programs for seniors and adults with disabilities, as well as resources and support services for family caregivers. Together, the DayBreak network addresses health concerns from the perspective of holistic wellness and prevention. As the organization matures, our essential mission and activities have maintained focus on two areas: 1) Ensuring broad access to care through information, planning and advocacy; and 2) Strengthening the depth and quality of care provided in local centers through program initiatives, including falls prevention, medication safety, and caregiver support. Each year, DayBreak and its member centers assist over 5,500 seniors, adults with disabilities, and family caregivers. DayBreak also serves as an expert resource for social workers, physicians, discharge planners, and other professionals — providing the information they need to find the right programs and services for their clients.Looking forward, we hope to continue expanding our ability to serve as a community resource for everyone to age well with independence, and with dignity.
