Health Heritage was launched in May 2014 at NorthShore University HealthSystems and was acquired by NantHealth, LLC in October 2015. It has three unique capabilities aimed at the intersection of genomic and personalized medicine: First, it provides consumers with the ability to easily connect with their electronic medical records and use a custom built natural language based processing engine to extract specific detailed medical data. These include the number, type, and location of colonic polyps and the exact diagnosis and dates of onset of significant conditions that are needed to determine their risk of developing important common diseases such as cancer. Second, it has dynamic methods for family members to share these data to create accurate family medical histories. Third, it has an extensive risk assessment engine based on the latest scientific evidence and clinical guidelines that uses these data to provide individualized prevention and screening recommendations ranging from changes in lifestyle to referrals for genetic testing.We are the only family medical history application that connects directly to a person's electronic medical record (EMR) and automatically extracts key clinical data needed to personalize prevention and treatment decisions. We do this in a way that is independent of any one specific EMR platform. This liberation of data can have profound implications for ownership and control of personal medical information. We have also transformed over 250 pages of detailed and complex clinical guidelines plus scientific literature regarding the identification of individuals at risk for common conditions and their most appropriate management into hundreds of algorithms that produce personalized recommendations. Finally, we have established a unique family sharing function that enables the creation of evolving and complete electronic family medical histories.
