CASSE Community Health Institute's mission is to provide personalized, high-quality care on an as-needed or preventative basis. We have created a practice that we believe in and choose for our own family members.We are an FQHC with a Patient Medical Home philosophy. We also believe in educating our patients as we work with them to achieve their best physical state possible.The Council for the Advancement of Social Services and Education (CASSE) is a 501(c)3 non-profit organization that identifies, designs, implements, coordinates, and evaluations programs in order to improve the quality of life in the communities served with dignity and respect by providing a continuum of care.CASSE is always looking for dedicated team members!
