NCHC encompasses six community health centers - including a walk-in primary location, three dental centers, , and a Medicare-certified home care and hospice division. The community health centers are NCQA recognized as patient-centered medical homes. NCHC is a nonprofit corporation, established in 1976 by community residents concerned about their poor access to health care and has a rich history and tradition of innovation in caring for our friends and neighbors.
