We are in Crestview, Quincy, Wewahitchka, Eastpoint, Tallahassee, Panacea, Madison, Perry, Mayo and Cross City. Most recently, we have added a mobile clinic, Health Force One. Our corporate offices support our clinics and are based in Tallahassee. Our clinics are Federally Qualifing Health Centers and as such can offer a sliding fee program in addition to taking insurance (including Medicaid and Medicare). Our current slide fee program begins with a nominal fee as low as $10.
