Care Support of America's (CSA) proven Advanced Illness Coordinated Care® service is contracting with the country's leading payers to provide health counseling and health education for patients facing advancing illness.CSA's Advanced Illness Coordinated Care® (AICC™) service helps patients and their families cope with serious chronic illness. AICC™ has been proven to improve quality of life and reduce unnecessary hospitalizations.What CSA Provides An experienced Registered Nurse provides each patient with three months of telephone based health counseling and education, followed by up to nine months of monitoring. If necessary, a community-based nurse may visit the patient's home to assess safety. CSA integrates seamlessly with health plan case managers and physicians and most importantly, family caregivers, to provide a coordinated advanced illness service.
