Primary PartnerCare® is a physician-driven organization with doctors in Nassau, Suffolk, Queens, and New York Counties. We were founded on the belief that private practice primary care doctors in partnership with their patients produce optimal health. Established in 2012 to help independent physicians meet the demands of healthcare reform, we provide tools and resources for doctors to remain independent and thrive.Effective January 2014, one of our independent physician associations, Primary PartnerCare Associates IPA, was selected by CMS to participate in the Medicare Shared Saving Program. As participants, our doctors are able to collaborate and provide additional care coordination and support programs for their patients.
