When multiple healthcare providers from different professional backgrounds provide comprehensive services by working with patients, their families, caregivers, communities, and other health workers , new possibilities exist that were not there before. The Collaborative Healthcare Network is a diverse multidisciplinary team of health specialists focused on restoring health, function, and wellness in an effective, efficient, and collaborative manner to our community.Modern health care is embracing collaborative care as multiple research has demonstrated that it strengthens health-care systems and improve patient outcomes. When a person's health and wellness are the priority we acknowledge that one healthcare professional may not provide all the answers.
