PLAN of PA (Planned Lifetime Assistance Network of Pennsylvania) is a non profit organization providing Care Management support and Corporate Trustee Services to assist people with Mental Health and Intellectual Disability diagnoses to live happier, healthier and more meaningful lives. With a focus on the person and his or her support system, we offer expertise, support and guidance to achieve current goals while planning for the future. Our Special Needs Trust fiduciary services provide a range services. We often assist families and individuals in setting up a trust, which can be intimidating at first. As a corporate fiduciary, with engage the person, his or her network of support in creating a Trust Plan, which is a pragmatic goal oriented action plan to carry out the intentions of the trust. We regularly engage legal experts, tax professionals, financial advisors. medical professionals, insurance providers and various additional community professionals to assist the beneficiary in utilizing trust resources for his/her benefit. We act as a Corporate Fiduciary for 1st party and 3rd party Special Needs Trust and we also offer a Pooled Trust. Our Trust Services are uniquely positioned to integrate Care Management services when indicated. Our Care Management services assist people to find housing, obtain public benefits, maintain wellness, coordinate medical/ mental health care, seek out employment, find volunteer work, pursue an education, maintain/develop solid social connections and manage personal finances.
