Council Bluffs Community Health Centers began in 1997 as a clinic located at Place 35 in the west end of Council Bluffs. In 1998, as the need for services grew, the clinic was relocated to the Omni Center as a Federally Qualified Health Center (FQHC). Since then a Dental clinic, X-ray services, a handicapped accessible van and translation services have been added to eliminate barriers to health care for the entire community.\\Mission Statement: Council Bluffs Community Health Center provides accessible, comprehensive, culturally competent primary health care to the entire community, embracing the opportunity to help those with limited resources and other barriers to quality health care.\\CBCHC accepts Medicaid, Medicare, private insurance, and offers a sliding fee scale for the uninsured and underinsured (based on eligibility).
