We built the first AI-driven, behavior shaping platform that offers remote digital care journeys for patients with multiple chronic conditions - all of the same platform. This omnichannel solution creates micro-interventions tailored to individuals. In other words, it offers a choice of health actions and nudges patients towards those they are motivated to do and able to do in managing their health.
