Optima Health has been selected to contract with the Virginia Department of Medical Assistance Services (DMAS) to provide Medicaid managed long term services and supports to Virginians with complex health issues. Eligible individuals will be enrolled in the new statewide Commonwealth Coordinated Care Plus (CCC Plus) Program.CCC Plus will build upon the foundation of Virginia's current Dual Demonstration Program: Commonwealth Coordinated Care (CCC). CCC Plus will provide services to approximately 214,000 individuals including dual eligible (Medicaid and Medicare A and/or B) members, individuals receiving long term services and supports through a nursing facility or a home and community-based services waiver, as well as Virginia's aged, blind, and disabled populations.The CCC Plus program will operate as a fully-integrated delivery system with a comprehensive continuum of care covering physical health, behavioral health and addiction and recovery treatment services, community-based supports and services, and institutional services with very few services carved out. Through person-centered care planning, Medicaid managed long term services and supports health plans will ensure members are aware of and can access community-based treatment options designed to serve them in the settings of their choice.Optima Health has designed a new product, Optima Health Community Care, to serve these members in their homes, healthcare facilities, and communities. Optima Health Community Care will provide high-quality, comprehensive, coordinated care to Virginians requiring managed long term services and supports statewide.Enrollment in CCC Plus will be phased-in by region beginning with the Tidewater region in August 2017.For additional information about CCC Plus, please visit the DMAS website.
