PHA partners with communities, hospital systems, healthcare organizations and practice providers to improve patient care. We focus on chronic disease management, opioid epidemic response, long-term care transitions, tobacco intervention, telehealth optimization and value-based care. Through fee-based contracts and no-cost programs, we consult, coach and train healthcare professionals in lean process improvement, IT security for healthcare, metrics reporting, and community building through Strategic Doing. We have partnered with multi-state coalitions to provide services on several grants.
