SFACHN clinics are dedicated to Patient Centered Medical Home (PCMH) principles and practices.In 2015, Community Health Network elected to develop an open source electronic health record and modifying it to support the needs of an FQHC. Community Health Network partnered with Williams Medical Technology in this endeavor which required significant additions to the system; including, design, documentation, reporting, workflow, and billing. Community Health Network key staff members and Williams Medical Technology programming staff meet at least weekly to further the development of interoperability, clinic workflow, decision support systems, data warehouse and patient communications.
