This is accomplished through a person-centered plan to identify specific interests, support needs and choices. Individual plans are monitored daily and annually by a multidisciplinary team, the individual served and family or significant others.All of our programs reflect the mission of 21 Plus, Inc., person-centered plans and a philosophy of providing meaningful lives for people with disabilities, with an emphasis on community-inclusion.
