Community Health and Wellness provides private programming--therapies and classes as well as services to clients on three state Medicaid waiver programs:Youth Empowerment Services (YES) Waiver, Home and Community Services (HCS) Waiver, and Community Living Assistance and Support Services (CLASS) Waiver.WHERE WE SERVE.Our company sees clients all across Texas, from down to the Corpus Christi Bay all the way up to Dallas-Fort Worth.
