Partnership with DCF.•Hurricane emergency and communication planning: during hurricane season. •Case management: To provide advocacy and assist in accessing and coordinating services, resources and supports in the community to maximize consumer's independencePEER SUPPORT/INDEPENDENT LIVING SKILLSVIRTUAL Recreational and social opportunities: •Healthy Living and Financial education and other educational topics. •Support Group: learn and discuss different topics like self-advocacy, self-determination, Independent living etc. •Stretch, Balance and Breathe classes. EMPLOYMENT SUPPORT: •Resume writing, •Job search, •Referrals to employment supports, •Partnership with vocational rehabilitation and other employment supports, •SSA Benefits Counseling and Ticket To Work Orientation.
