In July 2015, the Australian Government introduced Primary Health Networks (PHNs) across Australia with the key objectives of increasing the efficiency and effectiveness of health services. North Coast Primary Health Network (NCPHN)'s focus is on people in our community at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place, from the right team. NCPHN’s six key priorities for targeted work are: 1. Mental health 2. Aboriginal and Torres Strait Islander health 3. Population health 4. Health workforce 5. eHealth 6. Aged care NCPHN works directly with general practitioners, other primary health care providers, secondary care providers and hospitals to ensure improved outcomes for patients.
