The Patient-Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means to build a reliable primary health care system that delivers accessible, affordable and high-quality health care. The medical home model is built on four general concepts and attributes for medical practice. It is Patient-Centered.
