Health to Hope is a Federally Qualified Health Center that provides health care to the homeless throughout Riverside County. We provide primary care (family doctor care), vaccinations, chronic disease management, substance abuse/mental health, case management, and other services. We also provide referrals for specialty and other care with local non-profits and County resources. Health to Hope provides services across Riverside County including: The City of Riverside, Moreno Valley, Temecula Valley, and the Coachella Valley. Our services are offered at 20+ locations where homeless people congregate, without regard to their ability to pay. Sites include homeless shelters, soup kitchens, streets, bridges and others. Care may be provided from on-site clinics, a fixed clinic at the Riverside Access Center or through the fully mobile "Health in Motion" RV, which has two exam rooms. With the help of our dedicated and compassionate staff, Health to Hope's mission is to provide hope and improve physical and mental health of Riverside County's homeless, uninsured, and underserved population.
