An individualized care plan will be created that includes recommendations, identification of resources and possible funding sources available to improve their situation and address a variety of concerns. Follow-up services include ongoing care management, care coordination, reassessment, ongoing monitoring, crisis intervention, patient advocacy, caregiver support, and consumer education. Other services offered by The Option Group, LLC include personalized senior transportation services and a variety of support services around the home to include installation of safety items or durable medical equipment , small handyman and home maintenance tasks, as well as packing, downsizing and move coordination services.
