You will receive a detailed plan of care on you're initial visit, which will explain all the services and treatments that we will be providing to you.You can expect the following:▷ We will start your care within 24 hours of you being discharged from the hospital or receiving orders from you physician.▷ We will review all your Medications.▷ We will complete a Full Health Assessment.▷ We will complete a Home Safety Assessment.▷ We will answer any questions you have.▷ We have a dedicated Registered Nurse on call 24 hours a day to answer any questions you may have. We will coordinate your care with your physician to ensure you are getting exactly what the doctor ordered. Your physician can login into our Physician Portal 24 hours a day to view the progress of your care.
