We deliver seamless continuity of care by working directly with New York MLTC, CHHA, and hospice companies. Our in-home and care programs include home health aide assistance, specialized memory care, post-stroke care, home infusion therapy, appointment and procedure escort, private duty nursing, hospice and palliative care, and consumer-directed assistance (CDPAP).
