Our Physicians and Our Community: The Chinese Community Accountable Care Organization (CCACO) is a community-based accountable care organization. CCACO is a physician-owned and operated accountable care organization. We are composed of more than 200 doctors, the majority of whom have practiced medicine for over 10 years. CCACO has over 100 primary care physicians, as well as specialist physicians in the areas of cardiology, gastroenterology, otolaryngology, endocrinology, gynecology/obstetrics, pediatrics, etc. The Board of Directors that leads the organization is composed of 11 esteemed physicians, each of whom is recognized within the community as an expert in his particular area of practice.CCACO will serve 12,000 Medicare Fee-for-Service beneficiaries in the Chinese communities in Manhattan, Queens and Brooklyn. We are committed to the education of our patients and want to partner with them in coordinating their health, working together to forge a truly patient-centric healthcare system.Our Mission: CCACO's mission is to meet our patients' healthcare needs, with the goals of simultaneously improving health, promoting positive patient experiences and reducing per capita costs. We are a clinically integrated group working together to seamlessly coordinate patient care, and we aspire to the highest levels of transparency and honesty. We wish to enhance the quality of healthcare for this population that has special health needs, unique medical profiles, and varying proficiency in English. We aim to do this by bringing together physicians who are uniquely engaged with the Chinese community, and familiar with its health and linguistic challenges.
