The Patient-Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means to build a reliable primary health care system that delivers accessible, affordable and high-quality health care. The medical home model is built on four general concepts and attributes for medical practice. It is Patient-Centered. The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced office hours and around the clock access by telephone to members of the health care team.
