Community based service offering self management 1:1 support for people experiencing health conditions with the aim of supporting people to move back into the labour market. CMP is funded by JobcentrePlus (Department for Work and Pensions). We are a multidisciplinary team of mental health nurses, occupational therapists and one health visitor.
