RecoveryPlus.health has created a breakthrough home-based cardiac rehab option for patients that are unable to attend a traditional program for transportation or other issues. The program's targets and parameters are based on the referring physician's prescribed care plan. Our multidisciplinary staff provides access to dietitians, exercise physiologists, registered nurses, and health coaches to meet each patient's needs. Using our platform there is no discharge timeframe or session limit. The patient can remain in the rehab program until end of life, patient volition, or discharge by the physician. The RecoveryPlus.health suite of products consists of an app-based rehabilitation platform, the patient-management dashboard and the admin console managed by RecoveryPlus.health. These tools are coupled with the billing approach that makes all services reimbursable by Medicare and private insurance.
