Vitalis supports insurers, hospitals, specialists, GPs, corporations, retirement villages and individuals with programs and services that promote self-management and preventative care, reducing avoidable hospitalisations. We also support successful transitions from hospital to home, reducing readmission rates and improving well-being, through the provision of telephonic nurse support and clinical services such as HITH, Chemo at Home and REHAB+.
