Adams Health Network includes Adams Memorial Hospital located in Decatur, Indiana,, multiple physician practices located in Decatur, Monroe, Berne, and Geneva, Indiana. Our network also includes two Long-term Care Facilities (Adams Woodcrest, and Adams Heritage) as well as an Assisted Living Facility called Adams Evergreen, which will be expanding in January of 2019., Adams Health Network provides patient centered care and our mission is to serve our patients with with compassion and excellence. Awards-- Both Adams Woodcrest and Adams Heritage have been recognized as 2016 recipients of the Bronze Commitment to Quality Awards for their dedication to improving the lives of residents through quality care initiatives. Adams Memorial Hospital consistently boasts high ratings compared to other regional hospitals in Northeast Indiana on medicare.gov.
