Rocky Mountain CARES (RMC) is a 501 (C3) non-profit organization established in 2009 with the mission to provide the highest quality individualized comprehensive care, treatment, supportive services and education for the those affected- and infected by HIV in the Denver metropolitan area. The clinic staff is highly experienced in HIV care and already serves a large population of individuals. This application proposes to meet gaps in local HIV/AIDS care including delayed access to counseling and initial care, linkage to ongoing primary care, insufficient case management services and fragmentation of care. These factors are known to lead to delayed diagnosis, increased morbidity and healthcare costs. By providing testing, counseling, medical care and case management in a single physical facility, we hope to streamline the process of delivering comprehensive HIV services, while improving efficiency and decreasing costs. By aggressively increasing the visibility of HIV testing in key points of access, particularly in at-risk populations, we will improve the delivery and coor-dination of HIV services for the communities of greater Denver area. We aspire to be a model of efficient and effective care that will raise the standard of HIV care throughout the Intermountain West.
