aShorterPath's mission is to improve patient accountability and to meet this challenge we have developed the first Social Patient Health Record (PHR). Today, patient health knowledge is disconnected, siloed within each hospital and doctor's office, with little collaboration across health providers. Patients are left isolated, even family and friends are blocked from basic health information like insurance contacts and doctor's notes. To get the most out of our health care system, patients must take control. The first step is socializing patient health care information with family & friends. Care is a collaborative process, as in business and sports, teamwork matters. When family & friends are informed and share accountability for care, patient's health care decisions, their outcomes and the overall experience improves.
