Sustained Hope, LLC, an early-stage homecare company, has SPRINT, a new chronic care platform for post-discharge management of brain and heart trauma patients. SPRINT seeks to enhance patient outcome (i.e., eliminate frequent readmissions and improve health and quality of life) while reducing cost of care. Post-discharge patient follow-up, care delivery and evidence-based care planning are absent or inadequate at best. Significant disparities in care delivery prevails across the entire care continuum. Care centers are fragmented; they operate as silos with zero-sum competition, producing little or no meaningful value to the patient. Costly long-term complications and readmissions frequently result from inadequate or nonexistent follow-on care after discharge. Together, these devastating diseases cost over $300 billion annually in care burden in the USA alone.SPRINT is a post-acute careflow management platform, with integrated technologies and devices for: real-time patient assessments; HIPAA-compliant patient records (EMR & EHR) integration; remote care telehealth monitoring and on-demand reporting of patient data and analytics. Digitized assessments, vitals, medication adherence; remote sensing of patient motion/activity are exemplary technologies integrated into SPRINT. To ensure accuracy and efficiency, SPRINT is maintained and managed by a specially trained Patient Navigator, who ensures care coordination and integration through 3-way bidirectional communications among all stakeholders, as shown in the figure on right. SPRINT is specifically designed and developed to identify and eliminate barriers to care delivery, whereby providing superior value (better outcome at reduced cost) to providers, payors, care centers and patients alike.
