NPalliative®️is a member of the American Academy of Home Care Medicine (AAHCM) and Center to Advance Palliative Care (CAPC). We are licensed practitioners in Northern Michigan that assist in the healthcare management of those experiencing chronic disease. Our specialties include: palliative care, COPD, CHF, dementia, disabling illnesses, cancer, care coordination, transitional care, hospice, medication monitoring, life limiting diseases, counseling for grief, loss, & trauma, caregiver support, patient advocacy, decision-making, prevention of readmission, and especially symptom management.We are always hiring excellent practitioners who love autonomy, and are accepting new patients in Northern Michigan!
