Stacy's Helping Hand has a unique team of Interim Care Managers. Since 2007 Stacy's Helping Hand has been helping seniors find the care they need. Our Interim Care Managers meet with a family and their loved one listen to their concerns and needs. The in-person assessment is invaluable. If moving is necessary, we will begin the process of looking at care options. We will keep safety, care and comfort foremost in mind when choosing among the many senior housing options that meet a family's needs and budget. Many times care facilities are pre-toured. Once we have identified the communities that will best meet a senior's needs, we will schedule tours and accompany them. We are with a family every step of the way, answering questions as we go to each community. Once a community is chosen, we can assist with the necessary paperwork and transition. We are available afterwards to ensure that the care chosen is meeting the senior's needs for comfort and safety. We are in constant contact with the family and the facility for three months (2, 4, 8 and 12 weeks). Additional Services: * Assistance with Veterans Aid and Attendance application * Liaison and Advocate in care conferences * Review of Long Term Care Policy and assistance with approval * Support during guardian or fiduciary hearing * Assistance with Medicaid application Over the years we have helped hundreds of families find the care they need for their loved one.
