An experienced, professional case manager will assist families in making sound short and long-term medical, emotional, and financial decisions. A clear, individualized plan for coordinating health care will be drafted based upon your input, goals, needs and desires. Through disease management, we prevent unnecessary or repetitive medical care in conditions such as Depression, Dementia, Chronic Obstructive Pulmonary Disease (COPD) and Diabetes. Caregiver support and fall management programs are also available. We strive to ensure that the client receives all the necessary services to promote health and maximize well-being while helping the client remain independent.
