Alive-PD ("prevent diabetes") is a fully-automated algorithm-driven intervention for people at risk of developing diabetes. This includes persons with obesity or metabolic syndrome in addition to those with pre-diabetes. Alive-PD provides a one-year program of regular contact, including individually-tailored weekly goal-setting and other activities delivered via web and email, supplemented by automated interactive voice response (IVR) phone calls and a supportive smartphone application for Android and iPhone. The curriculum is based on established principles derived from several bodies of behavior change research. Alive-PD is the only "virtual" diabetes prevention program that has evidence from a randomized controlled trial. Others have a single-arm pre-post design. Alive-PD has evidence of effectiveness against a wide range of diabetes-related endpoints. Intervention subjects had outcomes significantly better than control for HbA1c, fasting glucose, BMI, weight, waist circumference, metabolic syndrome, insulin sensitivity and Framingham 8-year diabetes risk.
