Health Change Australia has developed a unique practice methodology that improves clinical practice and health service delivery. At the heart of the methodology is the HealthChange Decision Framework that enables clinicians and services to deliver patient-centred care, self-management and behaviour change support within their normal clinical consultations in a time efficient and effective manner. The methodology is used within all types of health service delivery and for all health conditions spanning prevention; treatment of chronic disease and rehabilitation from illness or injury. The methodology can be applied to any individual client or group setting where clients are required to take some action to achieve better health or quality of life outcomes. The aim is to increase the likelihood that clients will carry out lifestyle and treatment recommendations that are appropriate to their health condition/s and risk factors. Health Change Australia provides a range of consultancy and training services to health service managers including input relating to program design, quality assurance systems and audits and program evaluation. We are involved with a number of research initiatives with several universities in Australia. We have worked with most Australian government health services and similar organisations in Canada.
