Home Medicine USA Inc. [HMU] is specializing in developing proprietary medical scoring tools, for evaluating multiple clinical parameter changes. HMU is developing "Clinical Intelligent Suite"- which converts HIT into a meaningful tool. A. Clinical Watch' [CW] - is an add on to any EMR to determine clinical trends for early detection of changes in clinical conditions. It is designed for the use within the health provider settings and the patient's HR. It is acting as a clinical seismograph, which reveals and follow sub-thresholds clinical deterioration tendency, taking in the account accumulated multi-parameter changes in the data inside the EMR. It reduces hospitalization rates, by flagging up accumulated small changes, which are meaningless separately [one by one], but might hint early yet significant change in the patient's condition. CW enhances treatment accuracy and reduces medical errors rate [personalized Medicine attitude]. B. ‘Monitor24' - Automated patient follow-up system designed to reduce early hospital readmissions rate of complicated patients, such as Congestive Heart Failure patients. At discharge, the patient is enrolled in the Monitor24 service with an individualized questionnaire being built from the Monitor24 library of multilingual predefined questionnaires. During an automated daily phone call, the patient responds to the questions (voice recognition or touch tone). Using dynamic algorithms, Monitor24 analyzes patient responses and "scores" the difference between successive sessions, and the baseline, according to the care team's pre-determined definitions. Monitor24 automatically sends a clear, concise professional report to the care team (via voice mail, e-mail, SMS, or directly to the EMR with flagging methodology). The report format transfers easily to the patient's EMR.
