The introduction of the Aged Care Reforms and the National Disability Insurance Scheme (NDIS) are driving the most significant shifts in what was the Home and Community Care (HACC) sector. The reforms commenced in Victoria in July 2016, starting with changes in the way consumers access services and who funds and oversees the implementation of services.The State and Commonwealth governments have committed to jointly fund sector development and change management support to enable providers to continue to work confidently within the NDIS and Commonwealth Home Support Program (CHSP) transitions, and to facilitate and encourage coordinated person-centred care and shared good practice.The North Metro Community Care Alliance (‘The Alliance') has been established within the local government areas of Banyule, Darebin, Hume, Moreland, Nillumbik, Whittlesea and Yarra, to provide support and strategic direction to the sector.
