They are skilled in conducting patient conferences, family and patient counseling and effective discharge planning to prevent costly readmissions.LONG TERM CARE & TRANSITIONAL CARE UNITSOur licensed social workers have experience in long term care facilities and transitional care units. They are able to perform:•MDS's, CAA's•Care plans•Care conferences•Admission paperwork•Discharge planningSCHOOLSOur licensed school social workers are certified to practice in schools and able to step right in to work with your students. They are well-versed in IEP work, crisis intervention and referral services to students and their families. They can also conduct formal and informal assessments and work effectively with a multidisciplinary team.HOME CARE UNITSOur licensed social workers are qualified to provide visits to home care clients in their private residence.
