They are skilled in conducting patient conferences, family and patient counseling and effective discharge planning to prevent costly readmissions.LONG TERM CARE & TRANSITIONAL CARE UNITSOur licensed social workers have experience in long term care facilities and transitional care units. They are able to perform:•MDS's, CAA's•Care plans•Care conferences•Admission paperwork•Discharge planningSCHOOLSOur licensed school social workers are certified to practice in schools and able to step right in to work with your students. They are well-versed in IEP work, crisis intervention and referral services to students and their families. They can also conduct formal and informal assessments and work effectively with a multidisciplinary team.HOME CARE UNITSOur licensed social workers are qualified to provide visits to home care clients in their private residence. We work with clients covered by Medicare, private insurance companies or private pay.COMMUNITY AGENCIESTemp Social Work provides social workers for many types of social service agencies including: care management agencies, adult day services, senior housing, assisted living, and block nurse programs.
