CareCentra's mission is to help people choose the healthiest possible action, in a connected, collaborative, compassionate way. We built the first AI-driven, behavior shaping platform that offers remote digital care journeys for patients with multiple chronic conditions - all of the same platform. This omnichannel solution creates micro-interventions tailored to individuals.
